When and Why Do You Need Rapid Palatal Expander (RPE) There are many dental and orthodontic appliances available to correct oral problems like misaligned teeth, jaw or narrow palate. Some of these procedures work the best when the patient is still young when the bones are soft, growing and easy to manipulate. One of the common dental appliances used by dentists on growing children is the rapid palatal expander (RPE). It is used to expand the palate and to correct the bite that either has a crossbite (“opposite” bite) or a narrow upper arch. It widens the upper jaw to create space for permanent teeth. RPE is usually worn for six to nine months in order to correct the bite. The American Academy of Pediatric Dentistry recommends correcting crossbites “as early as possible when behavior allows”, and at Smiles For Kids we can usually do it around 5 years of age. Young children are adaptable and flexible, and the procedure is smooth and easy for them. Since the joint of the upper palate is still non-convoluted, it is much easier to correct at this time. The RPE is an expander with two bands cemented onto last upper molars. They are connected by a metal screw that is turned daily to expand the appliance. Follow the dentist’s instructions regarding the turning schedule. We teach all our parents and caretakers how to turn the screw properly and easily. It usually takes two to four weeks to complete the active (or turning) part of treatment and following 6 months the appliance is passively providing retention. During the first two days, the patient may experience slight headaches and tightness, which is a normal effect. Over-the-counter pain medications will relieve any discomfort, but shouldn’t be needed for more than a couple of days. What to Expect Since the appliance is a foreign object, the patient may have difficulty speaking, chewing, eating and swallowing for the first few days, but after a couple of days children get used to it and do not experience any problems. Practicing speaking by reading a book aloud can help before the return to school. Soft diet will also help in those critical first days. To remove food particles trapped within the RPE, rinsing thoroughly with water after every meal and using a syringe that we will provide will help.

Braces Explained: Palatal Expanders In this video, I will be answer the question of "what are palatal expanders?" and "what are they used for?" Expansion can be used in several ways in orthodontics! The primary use of palatal expansion is in the younger patient who can achieve skeletal expansion where the patient has a posterior crossbite (upper back teeth are more towards the tongue than the lower back teeth). To correct this, we use a rapid palatal expander to expand the two halves of the upper jaw by using a jack-screw-like appliance. This is relatively painless and you feel some pressure but all-in-all it is well received. For the older patient, skeletal expansion can still be achieved to correct a posterior crossbite but surgery or mini-screws may be necessary to assist in separating the mid-palatal suture. When skeletal expansion is achieved, a gap will be noticed between the front two teeth, but rest assure that this gap will be closed once the braces are placed! Slower palatal expansion can also be used where we are trying to achieve a dental expansion rather than a skeletal one. To do this, similar appliances are used but the protocol of turning the expander is different & much less frequent/slower. Ultimately, your treating orthodontist knows best what you need and you should follow his/her personal protocol for you! In either case, expansion increases the amount of space in your upper jaw and allows for you to minimize the need for extractions in mild-moderate crowding cases! What are the different appliances for kids expanders and growth appliances. The ALF and Schwarz appliances are two appliances that are alternatives to rapid palatal expanders. There are so many orthodontic expanders available for kids. The best palatal expander is the one that is capable of promoting growth in the direction the child needs. Some appliances are also better at correcting certain conditions such as deep bite, cross bite and over bite. Other appliances may be more suitable for helping with pediatric sleep apnea and mouth breathing. In most cases it is better to do expanders prior to getting braces. Or in other words it is better to get the jaws to be the right size and shape and then straighten the teeth. More of the popular orthodontic expanders we use include twin block, homeoblock, bio block and of course ALF and Schwarz. We do not usually perform rapid palatal expansion or RPE. Braces Vs Expanders: Braces are more about straightening teeth and the goal of maxillary expansion is growing the jaws to be the a healthy size and shape. The optimal treatment in every case is to grow the jaws to be the proper size and shape and then align the teeth. Often times teeth will become naturally straighter after treatment with expanders. Creating enough room for teeth to fit can allow teeth to fit without crowding or being crooked. Usually orthodontics is necessary after expansion therapy, but in some cases an acceptable cosmetic result can be obtained with expanders alone. Schwarz Appliance stimulates Growth of the Jaws and Airway schwarz expander growth applianceThe Schwarz appliance is one of the most versatile facial growth appliances available. The appliance is a removable appliance worn 10-12 hours per day. The palatal expander should be put in after dinner and worn through the night. The Schwarz can be designed to promote growth of upper and lower jaws more forward, taller and wider. As the jaw bones grow, so do the surrounding facial bones and surrounding soft tissues such as lips, cheeks. The result is growth of the entire facial structures and the lower 1/2 of the face in particular. Warning signs that a child is not growing properly: mouth breathing especially at night dry lips and swollen tonsils crowded crooked teeth. This is the physical evidence that the mouth and nasal passageways are not growing optimally and something needs to be done or else. The Schwarz can be custom designed to stimulate bone growth where it is specifically needed. The picture above shows an upper appliance that can be adjusted forwards and wider. Additionally the back teeth have acrylic covering them. This is called a bite plane. The “bite plane” is shaped purposely to control how the lower jaw rests on it. The bite plane is usually shaped to create a more forward resting lower jaw position. A forward bite position stimulates the lower jaw to grow forward. The upper Schwarz is most often coupled with a lower complimentary device. lower schwarz applianceThe lower Schwarz only stimulates wider growth of the mandible. Schwarz appliances are very easy to tolerate and can be taken out during the day and during eating. The Schwarz appliance is Dr. Adams’ favorite appliances to correct crooked crowded teeth and night time breathing issues in kids. The end result of the treatment is more room for the teeth to fit, improved facial profile, larger mouth and nasal passageways, improved breathing and airway, and a larger nicer looking smile. How do Expanders and Growth Appliances Work: Palatal expanders work by placing light pressure in the direction where bone needs to grow. The natural action of muscles such as tongue pressure, cheek and lip movement will do the rest in most cases. Occasionally tongue and lip ties need to be released and myotherapy (tongue exercises) may be completed.
